Info for Contractors

NEW HIRES

Before you start your new assignment, complete the forms below and fax or mail them to the OTM office. We cannot release your first check, until we receive these forms from you.

• Employment application
• Federal W-4
• Federal I-9
  Note: Include photocopies of the required identification (typically, your passport or driver's license and your social security card or birth certificate).
• Emergency contact form
• Direct deposit form

You will also have to sign an employment agreement and NDA, which we will send you. Please contact us, if you have any questions.

TIME CARDS

Contractors are required to submit a weekly time card. Please fax or email your time card to the OTM office by Tuesday after the week you worked. If we do not receive an approved time card by 5:00 pm Tuesday, your paycheck may be delayed.
