Info for Consultants

REQUIREMENTS

If you have been hired as an independent consultant, you must satisfy the IRS requirements for independent consultant status. Requirements include:

• A legal business name, articles of incorporation, and a business license
• A certificate of commercial general liability insurance, if applicable
• Workers' Comp insurance, if applicable

NEW HIRES

Before you begin your consulting assignment, complete the Federal W-9 form below and fax or mail it to the OTM office. Include photocopies of the required identification (typically, your passport or driver's license and your social security card or birth certificate).

You will also have to sign an employment contract and NDA, which we will send you. Please contact us, if you have any questions.

BILLING

Independent consultants are required to submit a weekly time card, as well as a semi-monthly invoice for services rendered. Invoices should include the following information:

• Your corporation's name
• Your taxpayer ID number
• Invoice number
• Number of hours worked, contractual rate of pay, and total amount due

Payment will be made per the terms of your contract following receipt of your invoice and supervisor's approval of your time cards.
